The department of Resident and Family Services is available to assist
residents and families beginning with admission process and continuing
on throughout their stay at the facility. We provide guidance and
support during the transition of placement in the facility whether your
stay is for short term or long term care. We also act as a liaison between
residents and/or families, facility staff members and the doctor to assure
all issues and concerns are addressed in a timely manner. This
department can assist families in understanding and applying for
insurance benefits.
Resident and Family Services will also work with the resident and
family during a short term stay to find the most appropriate discharge
plan. This can include discharge to a lower level of care or to the
community. Resident and Family Services will also make arrangements
for home care services and equipment needed to ensure a safe discharge
home.
